- The HPPH COVID-19 Booking Line has closed as of May 31, 2023. Please remove any posters you have in your offices that have the HPPH booking line information. Beginning June 1, 2023 clients are asked to contact the Provincial Vaccine Contact Centre by calling 1-833-943-3900 or visiting the COVID-19 vaccination portal online at www.ontario.ca/book-vaccine/.
- COVID-19 Vaccine Receipts - To log in and get your receipt, you will need:
- a green photo health (OHIP) card (you will need numbers from both the front and back of the card, expired cards will be accepted)
- your date of birth
- your postal code
- If you have a red & white health card or no health card, call the Provincial Vaccine Booking Line at 1-833-943-3900. The call agent can email you a copy of your receipt.
