Patient Rostering Policy

 

The Maitland Valley Family Health Teams mission is to optimize the health of our community through collaboration and patient centred care. Our vision is to be a comprehensive and accessible primary care centre of excellence for our diverse community. 

If you are seeking a primary care provider, you may request to be placed our waiting list.  To be placed on the list you must live in immediate area, and not be currently rostered to a primary care provider.   

To ensure equality for all persons waiting for a provider, new patents are rostered from the patient waiting list on a first come first serve basis. 

 As there are so many individuals without a primary care provider in our community, changing primary care providers within the Family Health Team is not an option available to patients. Should you choose to not be a patient of the provider you are rostered with you may advise us in writing, and we will place you on the patient waiting list for the next provider with capacity for a new patient.  

Physicians that are members of the Maitland Family Health Team are independent practices and responsible for those patients rostered under their care.  Physicians’ practices do not report to the Maitland Valley Family Health Team.

We encourage you to actively interact with your primary care provider with regards to your health care, however our employees and health care providers have the right to work in an environment free from offensive or harassing behaviour. Should your relationship with your primary care provider be discontinued due to harassment, bullying, inappropriate or offensive behaviour we reserve the right to not place you on our patient waiting list.
